Job Description:

A company is looking for a Clinical Care Specialist to manage chronic care for patients in long-term care facilities.



Key Responsibilities

Coordinate and implement Chronic Care Management (CCM) services for chronically ill patients
Create and update individualized Care Plans while monitoring labs, medications, and testing
Conduct care coordination with patients, families, and the healthcare team through various communication methods


Required Qualifications

Active RN license in applicable state with compact license
Five years of nursing experience required
Experience with chronically ill patients preferred
Telephonic case management or care coordination experience preferred
Strong computer skills and reliable internet connection required
